Peak years like 2018 and 2019 saw around 2,300 to 2,400 confirmed cases nationally during the main season. Michigan alone is now reporting more confirmed cases than the entire country reported in some past years, underscoring how unusual this cluster is.

The challenge is not lack of effort. Investigators have interviewed roughly 900 patients so far, asking detailed questions about what they ate, where they shopped, and when symptoms started.
Michigan’s public health laboratory is also performing advanced genetic fingerprinting and whole-genome sequencing on human samples and any suspect produce they can find, hoping to match parasite strains and narrow the search. These are serious tools, and they reflect a real commitment to pin down the origin. Yet the investigation is fighting a built-in delay.
Cyclosporiasis can take up to two weeks after exposure for symptoms to appear, which means people are trying to remember salad choices from days or weeks earlier, often across multiple stores and restaurants.
Memory blurs, receipts vanish, and even the most careful interviews leave gaps. No recall has been issued, and officials admit they still cannot tie the outbreak to a single farm or factory.
National surveillance cuts and complex food chains deepen the mystery
The source problem is not unique to Michigan. Over the past decade, federal and academic reports have repeatedly warned that the United States lacks validated molecular typing tools to link Cyclospora found in patients to specific food sources.
That handicap has derailed more than one major cyclosporiasis traceback attempt, even when hundreds of people were sick and investigators strongly suspected imported produce.
To make matters worse, Michigan’s outbreak is unfolding after the Centers for Disease Control and Prevention reduced some foodborne illness surveillance in 2023, including removing Cyclospora from one of its key tracking networks due to resource decisions.

On the ground, the investigation also has to navigate a sprawling modern food supply chain. Fresh produce eaten in Detroit or Grand Rapids may have been grown in another state or country, shipped through multiple distributors, and repackaged under different brands before it hits a local shelf.
That complexity makes it harder to connect scattered cases across 26 or more states from Maine to Oregon, even though Michigan remains the clear hot spot.
Some consumers are already voting with their feet, shifting away from big chain stores and toward local growers they trust. Their instinct aligns with a broader skepticism toward remote, opaque systems.
When regulators and large distributors cannot quickly explain why thousands of people are sick, people naturally look for shorter, more transparent supply lines.
For now, officials advise simple, practical steps: cook produce when possible, wash salad ingredients under running water, clean cutting surfaces, and see a doctor if sudden, persistent diarrhea occurs.
These modest actions cannot solve the surveillance gaps or fix supply chain opacity, but they can help families reduce personal risk while the search for the true source continues.
